General instruction for Mobile App/IVR/ Online OPD Registration System

1. This mobile application is to be used for generating Online OPD registration Card for availing outpatient services at Shri Dada Dev Matri Avum Shishu Chikitsalaya.
2. You may bring the printout of OPD registration slip generated to the hospital atleast 30 min before the time given on the day mentioned on the card and proceed to the registration counter for token.
3. If not able to bring printout of OPD slip, printout of OPD registration slip will be issued at registration counter on showing of Bar Code / OPD Registration number in mobile phone and token will also be issued.
4. The actual time of the doctor consultation will be as per the token number issued to the patient and the number of patients waiting in the queue already with token.
5. In case the beneficiary opts for a wrong department / referral to other Department is required, he/she will contact at the Helpdesk for further help. The same slip will be valid for the referral.
6. This slip will not give entitlements to any preferential treatment.
7. For every revisit please login as registered user and generate a new OPD ticket.
8. Registration will be subject to actual availability of the concerned Doctor at the time of patient visit at the Doctor's chamber. The Hospital administration or authorized officer may allot alternative Doctor or refer the patient to other hospital.
9. Document proof uploaded should show the beneficiary name and ID.
10. The registration is for the date booked and is non-transferable.
